Posterisan forte ointment is indicated for topical treatment of symptoms of hemorrhoids, eczema and anal fissures.
Structure
1 g of ointment contains (active substances):
- 166.70 mg of a standardized suspension of bacterial culture (contains 5.0x108 E. coli killed and preserved in 3.3 mg of liquefied phenol);
- 2.50 hydrocortisone.
Excipients: lanolin, soft yellow paraffin.

Pharmacodynamics
The drug stimulates the processes of local immunity due to antigenic action, causing both non-specific and specific immune responses, resulting in increased local immune resistance. Accelerates and improves wound healing, has anti-inflammatory effect, prevents reinfection and superinfection.
Pharmacokinetics
Due to sorption or transcytosis, the components of the suspension of bacterial culture act as antigens and initiate the process of local immunization, which, in turn, causes a protective immune response.
Contraindications
- hypersensitivity to the active substance or to any component of the drug;
- specific skin conditions (tuberculosis, syphilis, gonorrhea) in the area of application, chickenpox, reaction to vaccination, fungal diseases, inflammatory skin diseases of the face (oral dermatitis, rosacea), other bacterial, viral, parasitic infections at the site of use, ulcerative lesions and atrophic changes in the place of application, anorectal tumor.
Mode of application
Before use, it is necessary to carry out hygiene procedures of the anal area. Adults apply ointment to the affected area of the skin and mucous membranes 2 times a day (morning and evening), if possible after defecation, gently rub with a finger. To enter the ointment into the anal canal and rectum, you can use the applicator with side openings, which is located in the package.
The duration of the course of treatment is determined by the doctor depending on the nature and course of the disease. Usually, the application period is 5-7 days.
Application features
Pregnant women
Corticosteroids cross the placenta. Currently, there is no clear information on the development of teratogenic effects in humans, similar to those observed in experiments on animals. With the systemic use of large doses of corticosteroids, effects on the fetus or newborn were reported (growth retardation in the uterus, adrenocortical suppression). Despite the small amount of information on the use of corticosteroids as an ointment during pregnancy, given their limited systemic absorption, it is possible to use mild to moderate corticosteroids (class 1 and class 2), such as hydrocortisone, for short periods of time and in limited areas of the skin. The above effects cannot be ruled out with prolonged use or application to large areas of the skin. This can only be done with special indications.
Hydrocortisone can be used by women who are breastfeeding, only in conditions of short-term treatment and application to small areas of the skin. With long-term treatment or application to large areas of affected skin, breast-feeding should be discontinued.
Children
Do not use.
Overdose
No cases of overdose or intoxication have been reported. An overdose causes inhibition of the function of the hypothalamic-pituitary-adrenal system. In case of accidental ingestion of ointment (for example, by children), the gastrointestinal tract (abdominal pain, nausea) is possible. Long-term use in large doses enhances resorption and increases the risk of developing systemic effects of hydrocortisone, such as menstrual irregularities, increased blood pressure, slowed wound healing, muscle weakness, insomnia, and increased blood sugar.If symptoms of hypercorticism are observed, treatment should be discontinued.
Side effects
In some cases, patients with hypersensitivity to the components of the Posterisan Forte ointment may experience hypersensitivity reactions, including itching and burning, depigmentation, hypertrichosis, and dry skin. May cause contact dermatitis (for example, through hypersensitivity to phenol). With prolonged use of the drug in high doses, the following dermatological reactions are possible: dermatitis, eczema, delayed healing of wounds and cracks, the development of secondary infection, atrophy of the mucous membrane, atrophy of the skin, cortex oppression, expansion of capillaries (telangiectasia), the appearance of stretch marks on the skin (stretch marks) and steroid acne.
Storage conditions
