Miaser ® is an antidepressant drug that belongs to the group of piperazine-azepine compounds. In the chemical structure of mianserin (the active component of the drug), there is no side chain characteristic of tricyclic antidepressants, which causes the anticholinergic activity of the latter. Mianserin increases central noradrenergic neurotransmission by alpha 2 auto-receptor blockade and inhibition of reuptake of norepinephrine. The drug binds to serotonin receptors of the central nervous system. The antidepressant effect of Miaser ® is similar to the effect of other modern antidepressants. In addition, the drug also has a pronounced anxiolytic effect, which is important in the treatment of patients with depression associated with anxiety. The sedative effect of Miaseru®, associated with its effect on alpha 1 -adrenergic receptors and histamine H 1 receptors, is a significant reason for the use of the drug in sleep disorders during depression.
Miaser ® is well tolerated by patients, including the elderly and patients with cardiovascular diseases. When used in therapeutic doses, it practically does not have anticholinergic activity and, accordingly, does not interfere with the functioning of the cardiovascular system.
In case of an overdose, Miaser ® causes significantly less cardiotoxic effects compared to tricyclic antidepressants. The drug does not interact with sympathomimetic and antihypertensive drugs, the effect of which is due to exposure to beta-adrenergic receptors (betanidine) or alpha-adrenergic receptors (clonidine, methyldopa).

Dosage and administration
Miaser ® tablets should be taken orally, swallow without chewing, washed down with water.
The dose of the drug is determined by the doctor individually for each patient.
For adults, the recommended initial dose of Miaseru ® is 30 mg per day. The dose can be gradually increased every 3-4 days to achieve the optimal clinical effect. As a rule, the effective daily dose of Miaseru ® is 60-90 mg, the maximum is 90 mg.
Treatment of elderly patients begins with a dose of 30 mg per day. In the future, the dose of Miaser ® can be gradually increased. The maintenance effective daily dose of the drug may be slightly lower than the usual adult dose.
The daily dose of Miaser ® can be divided into several doses, but it is better to take the drug at a single dose in the evening, given its beneficial effect on night sleep.
Treatment with adequate doses of Miaseru ® should lead to tangible positive results within 2-4 weeks of therapy. If the reaction is insufficient, the daily dose can be increased. If during the next 2-4 weeks there is no positive effect, treatment with Miaser ® should be discontinued.
After the achieved clinical improvement, in order to maintain the positive effect of the treatment, Miaser ® should be continued for another 4-6 months.
Discontinuation of treatment rarely causes withdrawal syndrome.
Overdose
Symptoms of significantly exceeding the recommended doses of Miaseru ® are generally limited to a prolonged sedative effect. Cardiac arrhythmias, convulsions, severe arterial hypotension, and respiratory depression are rare.
Treatment. There is no specific antidote. Gastric lavage is recommended, followed by symptomatic therapy and the maintenance of the basic vital functions of the body.
