Pharmacological properties

The action of Fanigan fast gel is due to the components that make up its composition.

pharmacodynamics. Diclofenac is an NSAID with a pronounced anti-inflammatory, analgesic, and antipyretic effect. The main mechanism of action is the suppression of prostaglandin biosynthesis.

In case of inflammation caused by injuries or rheumatic diseases, the use of diclofenac leads to a decrease in the severity of pain, tissue swelling and a reduction in the period of renewal of the functions of damaged joints, ligaments, tendons and muscles.

Menthol irritates cold receptors. This is accompanied by a narrowing and a decrease in the permeability of capillaries in the place of its application, a feeling of coolness. Menthol causes a local distracting and mild analgesic effect.

Methyl salicylate is a derivative of salicylic acid and causes a local irritant effect. Causing skin receptor irritation, methyl salicylate leads to the creation and release into the blood of a large number of biologically active substances that regulate pain sensitivity. The substance P is released from neurons. A decrease in the accumulation of substance P in the nerve endings leads to a decrease in the severity of pain. In addition, methyl salicylate, which belongs to the NSAID group, inhibits the synthesis of prostaglandins due to inhibition of COX, which reduces edema and infiltration of inflamed tissues.

Flax seed oil, the main component of which is a-linolenic acid (ethyl ester of an unsaturated fatty acid), has an anti-inflammatory effect, antioxidant properties, and improves blood circulation at the site of application.

Pharmacokinetics The action of Fanigan Fast gel begins a few minutes after application to the skin. After 20-30 minutes, the gel reaches its maximum. When applied topically, less than 5% of the dose is absorbed, so the systemic effect of the drug is minimal.

Indications

Local treatment of myositis, fibrositis, sciatica, stretching of muscles and tendons, traumatic injuries of the musculoskeletal system, pain in muscles and joints during heavy physical exertion, inflammatory and degenerative diseases of the joints (osteoarthritis of the peripheral joints and spine), rheumatic lesions of the musculoskeletal system (bursitis , arthritis, tendosynovitis, tendonitis).

Application

For adults and children over the age of 14 years, apply 2–4 g of gel (strip 4–8 cm) with a thin layer on the skin and easily rub 2-3 times a day. the average daily dose is 10 g of gel, which corresponds to 100 mg of diclofenac sodium. apply the drug on intact skin, avoid contact with eyes and mucous membranes. Do not apply gel on open wounds.

After application of the drug, hands should be washed, if only this area has not been treated.

The duration of treatment is determined by the doctor depending on the nature and course of the disease, the effectiveness of the treatment.

The gel should not be used for more than 14 days with rheumatism and soft tissue damage, or more than 21 days in case of pain with arthritis, unless otherwise recommended by the doctor.

Elderly patients do not require dose adjustment.

Contraindications

Hypersensitivity to diclofenac or other components of the drug. the presence of a history of asthma attacks, urticaria, or acute rhinitis due to the use of acetylsalicylic acid and other NSAIDs. damage to the epidermis, open wounds in the field of application. last trimester of pregnancy.

Side effects

From the side of the immune system: hypersensitivity reactions (including urticaria), angioedema.

Use with caution concomitantly with oral NSAIDs.

The likelihood of developing systemic side effects with topical administration of diclofenac is negligible compared with oral administration, but are not excluded when using the drug on large areas of the skin for a long time.

Do not use Fanigan Fast gel with other agents containing diclofenac. The drug should be used with caution with peptic ulcer of the stomach and duodenum in the acute stage and its history, impaired liver / kidney function, impaired hematopoiesis, polyposis of the nasal mucosa.

Fanigan Fast gel is not recommended to be applied to damaged skin areas, preventing contact with inflamed, wounded or infected skin. Avoid contact with eyes and mucous membranes. The drug should not be swallowed. Fanigan Fast gel contains propylene glycol, which may cause mild localized skin irritation in some patients.

If any skin rashes appear, treatment should be discontinued. Fanigan Fast gel should not be used under an airtight occlusive dressing. In case of sprain, the affected area can be bandaged with a bandage.

Use during pregnancy and lactation. Clinical experience with pregnant women is limited, so the use of this drug is not recommended during pregnancy and lactation. Fanigan Fast gel is contraindicated in the III trimester of pregnancy due to the possibility of developing weakness of labor and / or premature closure of the ductus arteriosus.

If there are good reasons for using Fanigan Fast gel during breastfeeding, when the expected benefit of the drug, according to the doctor, exceeds the potential risk, the drug should not be applied to the mammary glands or large areas of the skin and should not be used for a long time (more than 1 wk).

Interactions

Since systemic absorption due to topical administration of the drug is very low, the likelihood of interactions is very low. however, possible interactions that are known for oral forms of diclofenac must be considered.

Overdose

An overdose is unlikely due to the low absorption of diclofenac into the systemic circulation when applied topically. in case of accidental ingestion, it should be borne in mind that 1 tube of 100 g of the drug contains the equivalent of 1 g of diclofenac sodium; in this case, the development of systemic adverse reactions is possible.

If the drug is accidentally swallowed, immediately empty the stomach and take an adsorbent. Symptomatic treatment with the use of therapeutic measures used to treat NSAID poisoning is indicated.
