Pharmacological properties

Ergocalciferol (24 - methyl-9.10 - secacholest-5.7.10 (19), 22 - tetraen-3b-ol) - a preparation of fat-soluble vitamin d2. regulates the exchange of phosphorus and calcium, promotes their absorption in the intestine, distribution in the body, deposition in the bones during their growth. the specific effect of the vitamin is especially evident in rickets.

Indications

Prevention and treatment of rickets, as well as bone diseases caused by impaired calcium metabolism (various forms of osteoporosis, osteomalacia), parathyroid gland dysfunction (tetany), some forms of tuberculosis, psoriasis, lupus lesions of the skin and mucous membranes.

Application

Inside. in the treatment of rickets of the 1st degree in full-term children, the daily dose is 10,000-15,000 IU. for the course of treatment (30–45 days) - 500 000–600 000 IU. in the acute course of the disease, the indicated course dose is prescribed within 10 days. course dose for rickets of the 2nd degree is 600 000-800 000 IU. the course of treatment for this form of the disease is 30–35 days (subacute course) or 10-15 days (acute course). with rickets of the 2-3rd degree, repeated anti-relapse treatment is carried out at a course dose of 400,000 IU, which is prescribed for 10 days. for the treatment of rickets of the 3rd degree, the course dose is 800,000-1,000,000 IU. the course of treatment for this form of the disease is 40-60 days (subacute course) or 10-15 days (acute course). treatment of patients with severe acute rickets (including with concomitant pneumonia) is carried out only in a hospital, prescribing a dose of 800,000 IU for 3-6 days.

To prevent rickets in newborns and infants, ergocalciferol is prescribed for pregnant women and women during breastfeeding. During pregnancy (term 30–32 weeks), the drug is prescribed in low doses for 10 days (total dose - 400,000-600,000 IU). During breastfeeding, from the first days of feeding, an initial dose of 500 IU / day is prescribed. For prophylactic purposes, full-term infants are prescribed the drug from the 3rd week of life (about 300,000 IU per course). From the 2nd week of life, a total dose of 600,000 IU is prescribed to premature and artificially fed babies, twins, children living in adverse environmental, climatic and domestic conditions. In the prevention of rickets, ergocalciferol is prescribed according to one of the following schemes: “fractional” method - daily at 500–1000 IU for 1 year of life; method of “vitamin shocks” - 50,000 IU once a week for 8 weeks; “Compacted” method - for premature infants and children with frequently occurring intercurrent diseases, prescribe 300,000–400,000 IU of the drug for 10–12 days. After treatment with the drug for a year, ergocalciferol is prescribed to the child until he reaches 2 years of age in the autumn-winter period. In regions with long winters, prophylaxis is continued until 3 years of age.

The use of the drug in some forms of tuberculosis, psoriasis is carried out in accordance with complex treatment regimens for these diseases.

The daily dose for the prevention of tetany is 1,000,000 IU. With lupus erythematosus in adults - 100,000 IU, for children under 16 years of age - after eating at a dose of 25,000-75,000 IU / day in 2 divided doses. The course of treatment is 5-6 months.

Contraindications

Peptic ulcer of the stomach or duodenum, other diseases of the digestive tract, acute and chronic diseases of the liver, kidneys, organic heart diseases, active forms of tuberculosis, hypercalcemia.

Side effects

In case of overdose and the use of high doses of the drug, manifestations of hypervitaminosis d are possible - anorexia, nausea, general weakness, headache, sleep disturbances, irritability, hyperthermia, in the urine - leukocyuria, proteinuria, cylindruria; hypercalcemia, hypercalciuria, calcification of soft tissues, lungs, kidneys, blood vessels.

Since ergocalciferol is able to accumulate in the body, the drug is used under the supervision of a doctor, including a study of calcium in the blood and urine.prescribed with caution to the elderly and pregnant women over the age of 35 years. when using the drug in high doses, it is simultaneously necessary to prescribe retinol (10,000-15,000 IU / day), ascorbic acid and B vitamins.

Interactions

With simultaneous use with calcium salts, the toxicity of vitamin d2 increases. when prescribed with iodine preparations, vitamin d is oxidized. when combined with antibiotics (tetracycline, neomycin), ergocalciferol absorption is disturbed. combination with inorganic acids leads to inactivation of the drug.
