Pharmacological properties
Cardiac glycoside contained in the leaves of digitalis woolly. It has a pronounced positive inotropic effect, increases the systolic and stroke volume of the heart, lengthens the refractory period, slows av-conduction and reduces heart rate. It has a moderately pronounced diuretic effect, is rapidly excreted from the body, cumulation is less pronounced than with the use of digitoxin.
Indications
Congestive heart failure of the i – iii stage; preparation of patients with heart disease for surgery or delivery; paroxysmal supraventricular tachyarrhythmia (tachyarrhythmic form of atrial fibrillation, atrial flutter, supraventricular paroxysmal tachycardia).
Application
Inside, on the 1st day, 0.25 mg 4–5 times a day, on subsequent days, 0.25 mg 1-3 times a day (maintenance doses). Digoxin is administered intravenously at 0.25-0.5 mg (1-2 ml of 0.025% solution 1-2 times a day). the contents of the ampoule are diluted in 10 ml of 5% or 10% solution of glucose or 0.9% solution of sodium chloride and injected jet for at least 5 minutes.
Contraindications
Severe bradycardia, complete av-block, increased frequency of angina attacks, fresh myocardial infarction.
Side effects
Perhaps a slowing of the conduction (blockade) of the heart, bradycardia, extrasystole, lack of appetite, nausea, diarrhea; in children, in addition, deterioration, lethargy, anorexia. with the development of arrhythmia and other side effects, you should stop taking the drug.
