Pharmacological properties

The drug has anti-inflammatory, analgesic, angioprotective, antithrombotic effects. The mechanism of action of diclofenac is due to inhibition of the biosynthesis of prostaglandins, kinins and other mediators of inflammation and pain, a decrease in capillary permeability and a stabilizing effect on lysosomal membranes. nicotinic acid improves microcirculation, has an antithrombotic effect, promotes the activation of redox processes in tissues.

The pharmacokinetics of the drug is due to the complex action of the components. When applied in the form of a gel, diclofenac is absorbed into the skin very slowly, slightly and insignificantly (3–6% of the dose applied to the skin). With prolonged treatment or application to large areas, it gradually penetrates into the soft tissues, synovial fluid, enters the blood plasma. The concentration in the synovial fluid is quite stable, in the blood plasma it decreases rapidly. Diclofenac is metabolized in the liver by conjugation with glucuronic acid. With urine, approximately 70% is excreted, with feces - 35%. Niacin has a local effect.

Indications

Acute and chronic insufficiency of the veins of the lower extremities, post-thrombophlebitis syndrome, as well as rheumatoid arthritis, ankylosing spondylitis, acute gouty, infectious, psoriatic and post-traumatic arthritis, osteoarthritis, bursitis, tenosynovitis, sciatica, edematous-pain syndrome of traumatic etiology (traumatic etiology).

Application

The drug is applied 1–2 g (4–8 cm) 2–4 times a day with an even thin layer on the affected area, rubbing slightly into the skin. the course of treatment is 7-14 days.

Contraindications

Hypersensitivity to the components of the drug. peptic ulcer of the stomach and duodenum, ba, epilepsy, parkinsonism, mental illness, pregnancy and lactation; do not prescribe to young children.

Side effects

In some patients, local skin irritation phenomena can be observed - rash, itching, hyperemia.

special instructions

With caution, it is prescribed to patients with a history of liver, kidney, and digestive tract. Avoid getting the gel on wounds, as well as on the eyes and mucous membranes. in order to increase the effectiveness of therapy, the use of occlusive dressings is possible.

Interactions

Interactions of diclosan with other drugs have not been established. it must be borne in mind that diclofenac can potentiate the effect of anticoagulants, corticosteroids, lithium preparations, and also weaken the effect of furosemide and thiazide diuretics, increase the concentration of digoxin in blood plasma, reduce the tolerance of other NSAIDs, increasing their ulcerogenic effect on the gastrointestinal mucosa.

Overdose

General resorptive side effects characteristic of diclofenac are possible - vomiting, belching, discomfort in the epigastric region, diarrhea, headache, dizziness. symptomatic therapy is recommended, there is no specific antidote.
