Pharmacological properties
Advocate is an original combination drug. exhibits antianginal and anti-ischemic effects, has membrane-protective and antioxidant effects, improves myocardial metabolism, protecting cells from hypoxia, exhibits anti-atherosclerotic properties due to normalization of homocysteine metabolism.
The mechanism of action of Advocate is associated with the complex effect of the ingredients of the drug.
Magladen is more stable against enzymes than ATP preparations, which allows it to be taken sublingually. Magladen acts on purine (adenosine) receptors. Activation of purine receptors is accompanied by inhibition of the entry of calcium ions into cells, manifests itself as an anti-ischemic, membrane-stabilizing effect, antiarrhythmic effect.
Molsidomine and its active metabolites SIN 1 and SIN 1A by the release of nitric oxide contribute to the expansion of the coronary vessels. Unlike organic nitrates, addiction does not develop to the drug. Molsidomine has a positive effect on the performance of systemic hemodynamics, reducing the load on the myocardium. By blocking the synthesis of platelet phospholipases and activating the synthesis of prostacyclin by the vascular wall, Molsidomine exhibits antiaggregant properties.
Folic acid is involved in many metabolic processes. Of particular importance is the participation of folic acid in homocysteine conversions. By reducing homocysteine, folic acid exhibits an anti-atherosclerotic effect. By stimulating metabolic processes in the myocardium, folic acid improves its functional activity.
In conditions of coronary insufficiency, the drug exhibits an energy-saving effect by reducing the myocardial oxygen demand. Advocard reduces the amount of lactate in the myocardium, inhibits the activity of membrane-bound phospholipases, prevents structural and functional damage to the membranes of cardiomyocytes, ensures a constant quantitative and qualitative composition of membrane lipids.
Advocard improves coronary blood flow, increases exercise tolerance, reduces the frequency and severity of angina attacks.
Indications
As part of the complex treatment of coronary heart disease (stable angina pectoris of tension and rest, prevention and relief of angina attacks).
Application
Advocates tablets are taken sublingually, kept under the tongue until completely resorbed. single dose - 10–90 mg 3-4 times a day, regardless of food intake. the course of treatment is 20-30 days. with an attack of angina pectoris, 1-3 tablets are taken. the maximum single dose is 3 tablets of 30 mg or 9 tablets of 10 mg. the maximum daily dose is 400–600 mg.
Contraindications
Individual hypersensitivity to the ingredients of the drug. you should not appoint a Advocard for collapse and shock, a bout, a traumatic brain injury, during pregnancy and lactation. There is no clinical experience with the drug in children under the age of 14 years, so it can be prescribed to children only older than 14 years.
Side effects
Headache, hypotension, nausea.
special instructions
In elderly patients, increased sensitivity to the drug is possible, therefore, they begin treatment with a dose of ½ tablet 3 times a day. with normal tolerance of the drug, they switch to the use in usual doses.
Interactions
With the combined use of a Advocard with antihypertensive drugs (including nitrates), an increase in their effect is noted. concomitant use with sildenafil citrate is not recommended, since hypotension may develop.
Overdose
Overdose is accompanied by headache, dizziness, visual impairment. in these cases, the use of caffeine or caffeine-containing agents is indicated.
Storage conditions
In a dry, dark place at a temperature of 5-30 ° C.
